Your medical team will consist of the Hospice & Palliative Care certified Medical Director, a hospice nurse, and an aide. Your primary care physician may also be a part of your care team.
One of the most important aspects to hospice care is the emphasis placed on spiritual and social support. Emotional and spiritual support is always available to the patient and their family.
Our volunteers are trained to support patients and families during their hospice journey. Volunteers provide help with simple errands, caregiver respite, and most importantly, companionship.
One of the primary goals of our care plan is to ensure the comfort of the patient, and assist with medication and symptom management. We will check pain levels at each visit and make changes as necessary.
We understand caregiving doesn't stop at the end of a work day. Our after hours line is always open where you can call to reach a hospice nurse.
Grief support is built into everything we do, from beginning to end. We follow up with our families over a year after receiving our services and provide resources unique to their needs.
